Two clear liquids, two nearly identical vials, one word of difference on the label. Bacteriostatic water and sterile water for injection are not interchangeable, and the reason comes down to a single ingredient present in one and absent from the other. Once that ingredient is understood, the choice stops being a preference and becomes a property of how the vial is going to be used. The whole difference is 0.9% benzyl alcohol
Sterile Water for Injection, USP is exactly what the name says: water, purified and sterilised, with nothing added. No preservative, no buffer, no salt. Bacteriostatic Water for Injection, USP is that same water with roughly 0.9% benzyl alcohol dissolved in it — about 9 mg in every millilitre.
Benzyl alcohol is bacteriostatic, not bactericidal, and that distinction is the entire point. Bactericidal means killing organisms. Bacteriostatic means holding them from multiplying. The preservative does not sterilise whatever you put into the vial, and it cannot rescue a solution that is already contaminated. What it does is suppress growth in the small volume left sitting between one withdrawal and the next.
Bacteriostatic water vs sterile water, side by side
- Preservative. Bacteriostatic water has one; sterile water has none. That single line drives every other difference below.
- After the first puncture. A preserved vial is designed to be entered repeatedly. The convention for a punctured multi-dose vial is to date it at first entry and discard it within 28 days, unless the label gives its own figure. Sterile water carries no such protection: it is a single-use diluent, prepared and used, not stored for a second visit.
- Format. Bacteriostatic water is typically sold as 10 ml and 30 ml multiple-dose vials, sized for repeated entry. Sterile water appears in single-dose vials, ampoules and large-volume containers, sized for one preparation.
- Tonicity. Neither is isotonic. Sterile water labelling is explicit that it must be made approximately isotonic before use, because water without a solute given intravenously can cause haemolysis. Both are diluents — the thing they dissolve is what makes the final solution injectable, not the water on its own.
What the preservative buys, and where it is not allowed
What it buys is time across entries. A multi-dose window is only defensible when something is limiting bacterial growth in the residual liquid, and benzyl alcohol is that something. Take it away and the same vial is a one-preparation object, no matter how carefully it is handled.
Where it is not allowed is narrower and much firmer. Benzyl alcohol is contraindicated in newborns: the FDA acted on this in 1982 after fatal cases of what was named gasping syndrome in premature infants. The mechanism is metabolic, not allergic — benzyl alcohol is converted to benzoic acid, and a neonatal liver cannot conjugate and clear it fast enough. Preserved diluents are therefore kept out of neonatal preparations, and vials of bacteriostatic water carry that warning on the label. Individual sensitivity to benzyl alcohol also exists in adults, and some products and some routes of administration specify a preservative-free diluent outright. The product label and a qualified professional decide that, not a general rule.
What changes in the arithmetic, and what does not
The concentration maths does not care which water went in. Volume is volume, and the strength of the solution is the vial's total divided by the millilitres added.
As an example, purely to run the maths: a 5 mg vial reconstituted with 2 ml sits at 2.5 mg/ml whether that 2 ml was preserved or not. What the water changes is not the number — it is how long the number stays useful. With a preservative, a vial mixed at that strength can be planned around as a multi-dose object with a dated window. Without one, it is a single preparation, and the strength you calculated is only relevant once. That is also why the water type belongs next to the concentration wherever you work the numbers out, rather than in your memory of the day you mixed it.
Label the vial, because afterwards they look identical
Three facts, written on or attached to the vial, settle nearly every question that comes up later: which water went in, the date it was mixed, and the concentration it was mixed to. The first is the one people skip, and it is the one that cannot be recovered by inspection. A mixed vial gives up no clue about its diluent — not by clarity, not by smell, not by how it draws.
Buying the two together makes the mix-up easier, not harder: the boxes look alike on a shelf and the vials look alike in a drawer. Reading the full line on the label, every time, before the needle goes in, costs a second and is the only check that actually works.
