Where the 28-day number comes from

Twenty-eight days is the standard beyond-use date for a multi-dose vial after its stopper has been punctured. CDC injection-safety guidance states it plainly: date a multi-dose vial when you first enter it and discard it within 28 days, unless the manufacturer gives a different date for the opened vial — and never past the printed expiry on the label.

The rule exists because of the preservative, not the peptide. Bacteriostatic water is sterile water with about 0.9% benzyl alcohol added, and benzyl alcohol holds bacterial growth in check well enough that a vial can be entered repeatedly over weeks instead of once. Twenty-eight days is the point past which that protection is no longer assumed — after many needle entries, and with a little preservative lost through the septum each time.

Two clocks run at once. The microbial clock asks how long a punctured, preserved vial can still be trusted. The chemical clock asks how long the peptide molecule stays intact in solution. You are governed by whichever runs out first — usually the microbial one, which is why the date matters more than the look of the liquid.

How long does reconstituted peptide last in fridge storage?

For a preserved multi-dose vial kept cold, the working answer is weeks, with 28 days as the usual ceiling. Cold means 2–8 °C: a shelf inside the fridge, not the door, where the temperature climbs every time someone opens it.

Two things move that answer, and both are readable off the packaging:

Temperature is the lever with the most force behind it. A rough rule from chemistry is that every 10 °C of extra warmth roughly doubles the rate of chemical degradation. An hour on the counter while you draw is not a catastrophe; a weekend on a sunny windowsill is a different vial by Monday.

If a vial holds more than can reasonably be used inside its window, the laboratory answer is aliquoting: divide the solution into single-use portions in sterile vials, freeze them, and thaw one at a time so nothing is frozen and thawed twice. The powder form is the stable one. Lyophilized peptide kept cold and dark holds up far longer than anything already in solution, which is the practical argument for mixing a vial when you are ready to start it rather than when it arrives.

Whatever you decide, the vial should carry its own history: the date you mixed it, the water you used, and the concentration you mixed it to. Three facts, and the first is the only reliable answer to how old the solution is — appearance is not a test. Writing them on the label works; so does keeping them attached to the vial in the same place you worked out the concentration, so the mixing date is still there four weeks later, when it is the only thing you need.
